Personalized support for each child

We provide community social pediatrics care and services to children whose development is, or may be, compromised by difficult living conditions. We warmly welcome children and their families regardless of their needs, while respecting everyone’s culture.

Our medical practice allows us to share knowledge between all concerned parties to better understand the situation of each child and his or her family, in order to find and implement solutions together while respecting human rights.

Children at the heart of their adventure

Step 1

When a request is submitted to provide community social pediatrics care and services to a child living in the Côte-des-Neiges borough, the centre’s team guides them to the care and services best suited to their needs.

Step 2 

An initial meeting is organized with the child, their parents, as well as those who are significant to them, so that the centre’s team can assess their overall health needs. This team consists of a pediatrician, a psychosocial specialist or a social worker. The lawyer-mediator is also present to shed light on legal issues related to the child’s well-being. All parties are encouraged to identify sources of stress that affect the child’s development and help develop an action plan.

Step 3

An action plan is implemented to address the relevant needs and provide the care and services the child needs for optimal development. This plan focuses on the strengths of the child and their environment, and ensures their rights under the Convention on the Rights of the Child are protected.

Complementary activities

Various activities can be offered to children under clinical care based on their needs and seasonal availability. Ad hoc activities may also be offered during weekends, spring break and holidays, depending on available resources.
